Address Verification

Address Verification documents must be in the parent and/or guardian’s name, show the registering address, and be dated within 45 days of registration. Verification documents include at least one of the following:

  • Required: Mortgage Statement/Property Tax Receipt, or, Rental/Lease Contract Agreement or Payment Receipt
  • Required: Utility Bill (gas, electric, internet, water, or refuse)
  • Pay Stub
  • Voter Registration
  • Correspondence from a government agency
  • Declaration of Residency Executed by Parent or Guardian

Immunization Requirements

The following is provided by the California Department of Public Health

Students Admitted at TK/K–12 Need Records of:

  • Diphtheria, Tetanus, and Pertussis (DTaP, DTP, Tdap, or Td) — 5 doses
    (4 doses OK if one was given on or after 4th birthday. 3 doses OK if one was given on or after 7th birthday.)
    For 7th–12th graders, at least 1 dose of pertussis-containing vaccine is required on or after 7th birthday.
  • Polio (OPV or IPV) — 4 doses
    (3 doses OK if one was given on or after 4th birthday)
  • Hepatitis B — 3 doses
    (Required at admission to any grade except 7th grade​)
  • Measles, Mumps, and Rubella (MMR) — 2 doses
    (Both given on or after 1st birthday)
  • Varicella (Chickenpox) — 2 doses

The TK/K–12 immunization requirements apply to new admissions and transfers for all grades, including 7th grade, and students whose exemptions are no longer valid.​

Students Advancing to 7th Grade Need Records of:

  • TetanusDiphtheriaPertussis (Tdap) —1 dose
    (Whooping cough booster usually given at 11 years and up)
  • Varicella (Chickenpox) — 2 doses
    (Usually given at ages 12 months and 4-6 years)​

California schools are required to check immunization records for all new student admissions at transitional kindergarten (TK)/Kindergarten through 12th grade and all students advancing to 7th grade before entry. Parents must show their child’s Immunization Record as proof of immunization.

Records Release

If you are transferring your child from an existing enrollment with another school district, you will need to complete a Records Release Request form, and deliver it to your child’s school office for completion. To download the form, please click here.

Interdistrict or Intradistrict Transfer

Interdistrict and Intradistrict transfers are subject to approval by the District’s Board of Trustees.

  • If you are requesting an interdistrict transfer ‘in’ to Pioneer Union Elementary School District, please complete this form.
  • If you are requesting an interdistrict transfer ‘out’ of Pioneer Union Elementary School District, please complete this form.
  • If you are requesting an intradistrict transfer between Pioneer Elementary School or Frontier Elementary School, please complete this form.

Please Note: All applications are subject to approval by the District’s Board of Trustees. Applications are received and reviewed at regularly scheduled board meetings.
